This really depends on how the workflow is being started.

You could create a field in smartforms to store the comments.

You could create a sharepoint field that stores comments.

You could create a smartbox smartobject and store comments coming from a third party webform, infopath form...

The list goes on.  If you can provide some more details about where the workflow originates, where the comments will be entered, etc, we can probably provide more guidance.
