I am not receiving my notification e-mail.  Any suggestions as to why?

Created the first workflow everything works great, when i create the second one for a different task I set it up the same way, the workflow is firing but the notification e-mails are not being sent.

The To field has the same names as the workflow which worked correctly.

Just to make sure, the Task is being run within the workflow by viewing the Workflow History, correct?

I had to use Initiator instead of created by.
