K2 App for SharePoint in Form Based Authenticaiton Environments

Hi
We enabled forms based authentication on Portal.denallix.com web application on one of the hosted environments. With that we also enabled claim settings in K2 database (See the attached snapshot). In the sharepoint portal we can successfully log in wither as wondows user or form based user, but for the form based user the k2 worklist component still displays the Windows user (denallixadministrator) workitem. Also when we try to access K2 app for sharepoint it prompts for selection of Authentication mechanism (Sign in page), here if we select windows Authentication it works ok but for forms authentication it says "404 Not Found".
Please help, also let us know if there is a step by step configuration document available to setup K2 App for SharePoint in Form Based Authenticaiton Environments.
The environment is 4.6.7 K2 BlackPearl and Core 6.1.1 VM.
