Symptoms
One of our workflows is deployed to a Content Type in SharePoint and every time an item is created using that content type, two instance of the workflow is created instead of one.
Diagnoses
The issue was related to having two workflows firing for the content type.
Resolution
Going to the Workflow Settings for the Document Set, we noticed there were two workflows attached to the Set. Navigate to the Workflow Settings for the Document Set and set it to only have one workflow fire when an item is added. This resolved the issue.
