is it possible to make duplicate entries in the list using one form in responsive designer?

I  have 2 choices using the choice form control, one is personal information and then the other one is country.

 

  • for personal information, there is a textbox for name and age.
  • for country, there is textbox for name of the country and name of the city

 

when i choose personal information choice the country will be hidden. After I fill up all the textbox in the personal information choice. I will choose the country choice, personal information will be hidden and then I answer all textboxes on country.

 

After that, I will save them in the list but it must be on a different row.

 

can you help me @EuanGamble @emha ?

Hi @EuanGamble,

 

Please disregard the above example .

 

This is the new example that is simplier.

 

There are 2 types of ticket. One is Incident and the other one is for Request.

 

If i choose incident on a dropdown button, a "Score" text box will appear for incident. If i choose request on a dropdown button, a " score" textbox will also appear for request type.

 

I just want to save them both as individual rows of data on a list
