They have 1 blackpearl server and 2 sharepoint servers. The K2 for sharepoint component was not installed on both the app and web servers for the sharepoint server that faced the issue.
Resolution
Ensure that the K2 for SharePoint component is installed on both the web and app server in the sharepoint server.
