Symptoms
A SmartForm is to be used as a default for the New/Edit/Display form (set in the artifacts page for an Appified list/library). When opening Form Settings to set this, the SmartForm in question does not show up as an option in the drop-down.
Diagnosis
The form to be used was moved into a subfolder (located in the root folder for the list). Only forms that reside in the root folder will be populated into the Form Settings drop-down menus.
Solution
Verify that the necessary form(s) reside under the root folder for the list (this can be done from the Designer by navigating to the folder for the list). The form(s) will now correctly display in the Form Settings drop-downs.
