I havent faced this error before , but from wt i see this looks like ur form is referencing a workflow that doesnt exist , 

wt workflow process refereences u got in ur form ? and does the workflow process exist or is it deleted ?

if its delete it , u might wanna go to form's rules and delete the references , prolly rules like "action worklist" or "open worklist"

but if the workflow exists , u might wanna make sure to include it inside the package .
