Symptoms

What is currently occurring is that when a New Request form is initializing there is a control that doesn't surface as intended. Rules for the form have been checked and it doesn't appear that this control is being hidden in any manner.
 

Diagnoses

During initial troubleshooting it was discovered that the control in question was not surfacing the way that is was supposed to. This was checked against multiple browsers and the issue still occurred.
 

Resolution

After general troubleshooting, customer was asked if he could duplicate the exact same control on his view. After doing this the customer reran the form in question and the control showed up as intended. This may have been due to a rule change that occurred at some point and reapplying the control from the form fields corrected this.
