I can't speak for custom themes readily available for download, but as far as "custom" goes, custom themes are a breeze.  it's basically all CSS, and not that difficult to modify.

Changing an existing theme and making a small change here and there, like colors, font size, etc, *is* a breeze. More dramatic changes might required more time.

What helps a lot is a browser which shows you detailed infos about the css classes. FireFox helped me a lot, Safari seems to be even better, Chrome is also fine.
