Creating multiple list lookups to show information collected in another sharepoint list on a form.

I got this to work on the form designer using the preview option, but when i try and use the form in sharepoint the fields flicker. I suspect its because they share the connected to title? Any work arounds for this? My goal is to be able to look up a company from another list determine when they are preapproved and the instructions if they are.
