Diagnoses
This was caused by a broken form definition which resulted from an out-of-sync table control on the form. This is a very rare occurrence which may happen when merging cells on a form. Our labs provided a script to help resolve this.
Resolution
Labs provided a script to manually fix the XML definition which helped to resolve this
