Symptoms
Unable to package form due to missing references:
[Form] Form is referencing the [Activity] Activity that could not be found
Diagnoses
It turns out that PandD was picking up a different workflow which happens to be a duplicate of the actual process.
Resolution
We deleted the unnecessary workflows via Workspace and re-ran the PandD. After which, the package was successfully created.
