What version of Nintex Forms are you using (you can view this in the designer under the help menu) also let us know if its on-prem or O365 as well as Sharepoint version.

Thanks for replying but we were able to resolve this by upgrading our version to the latest update. The issue went away after the update activity. Thanks again for the support.
