Application Form not working in mobile app


I am new to K2 and I want to use the mobile application to surface a form. I created the form with the built in platinum theme. I checked the application form box under advanced advanced settings and saved the form. I moved the form to the System-> Application forms folder. I put in the url to this server on the K2 login screen on the mobile app. I log in and select forms from the menu. The application forms screen says "Sorry, something went wrong"

I'm not sure how to fix this error. Any help would be greatly appreciated.


The K2API site was configured with the incorrect application pool -- this was set to a default app pool that was running under a virtual service account.
We changed this to the correct App pool that ran under the K2 service account and users were able to access Mobile Forms.
