Symptoms : 

 I Tyr to create new Workflow via K2 Designer & I having an issue with specify the form in > Workflow Settings  > Form  >  Select form window.

Version installed on Server Machine:

K2 designer / Server : 4.6.9

Dot Net Framework 4.5

Silver light SDK 5 : 5.0.61118.0

IE :11

I am new to K2 Smartform dev &  don’t have any clue how to resolve this issue & what causes of issue.

Any help would be a great appreciated.
