Following an upgrade from 4.6.8 to 4.6.10
Whenever a workflow is run, trying to view a user's Workspace shows this error where the current worklist should appear, in red:
Item has already been added. Key in dictionary: '<Name of An Action'> Key being added: '<Name of An Action>'
In the logs there is this:

The corresponding Workflow compiles and deploys with no errors.
I cannot find any duplicate names, actions or outcomes in the workflow definition.
Any pointers or advice gratefully received.
