Diagnoses
The issue is when you have several tasks that require approval (e.g. 10-30), these need to be all actioned by one user. The user approves of these task but only a certain amount actually get approved, lets say 8 out of 10 or 20 out of 30 actually appear to be approved sending an approval confirmation mail to the user.
Resolution
The best way to get around this would be to would to make sure that all the servers configured on the farm or NLB are able to connect to each other. Also make sure that the Exchange message queues are in working order as well.
