I have a basic workflow which I have created many times before with no issues and all working fine but this one just keeps coming back with error. ERROR IN REQUEST APPROVAL ACTION OBJECT REFERENCE NOT SET TO AN INSTANCE OF AN OBJECT. It’s driving me crazy can anyone help me please ?

I have typically seen this when the form is referencing a variable or field that is not present in the form. This can easily happen when importing a form and forgetting to change the references.
If that isn't it then I would do test with a fresh form and see if you get similar errors, which should help narrow down if it's an environment problems or just a problem with this particular form.
