Task approval redirects to SharePoint task item instead of "ApproveReject.aspx" page

This is already happening few times to us.

Approval Task redirects to SharePoint Task item page instead of ApproveReject page, this suddenly happening to one of workflows which has a Request Approval module.

So has this already been fixed with any specific patching? Or any simpler solution or best practice has been advised by Nintex?
