Nintex Forms Missing Form Control Categories

In Nintex Forms, the only Form Controls category I have is General:

I noticed if I click the Browse tab on the ribbon, the SharePoint category appears, but I'm still missing the List Columns category:

Tried in IE, Chrome and (Chromium) Edge.

Anyone seen this issue? Any help would be appreciated! Thanks!

Does changing the resolution of the screen reveal the categories?
Is there a custom masterpage in use?
