Images are not being displayed when uploaded to a nintex form.

If we change the destination library when uploading the image then it displays, the images in the NewForm folder above also show as broken links from within SharePoint.

As the image is also showing as a broken link in SharePoint, this is more a SharePoint issue rather than a Nintex forms issue. As changing the destination folder works, it appears that this may be a permissions issue or an issue with the list/library where you are storing the images.
