Resolution
Verify the document which has the workflow running is not being checked out or edited by another user. If there is interference from other workflows or actions within the workflow itself, adding pauses can help rectify this conflict. If the check-in/check-out versioning feature is enabled, you can utilize the check-in/check-out workflow actions as well.
