Issue
When attempting to create a custom task form, the option is missing from the Nintex Workflow task action.
Resolution
Enable the following Features at the site collection level:
Nintex Workflow feature:
Nintex Workflow 2013 InfoPath Forms
Nintex Forms feature:
Nintex Forms for SharePoint List Forms
