Request Review custom email notification is not saving

We are currently using SharePoint 2013 and running Nintex Workflow 2013 (ver 3.1.5.0).

 

I am running into an issue where the changes I have made to the Request Review action's email notification is not saving.  I found a similar post from Glenda Segers (see thread "Empty message templates" - https://community.nintex.com/thread/1405) which was resolved by applying the latest build.  I followed the suggestion and applied the lastest build which brought us to the above mentioned version.  But this did not fix the issue.

 

Any suggestions on how to fix this would be greatly appreciated.
