How do I create a list that may have 50 different forms? Do I create a Form for each form and it shows in the list of documents under File / New Document ? How would I set up these forms?
You would need to setup multiple content types, and then create a Form for each type. You could in theory just add 50 "Item" content types to the list, and then go through and create a Form for each one.

While that's using 365, the same principle should apply to on-prem.
