Nintex Forms: Javascript stopped working, any suggestions?

We have a javascript that performs a typeahead, it has been working all through out until last week and does not work anymore. There are no changes made to the form, and the same javascript does work in our development environment. Any other javascript on the same form does work except this one. Any suggestions on what else can I do to troubleshoot?? Below is the script. Thank you!
