Is there any way that user can enter comments when they reject an approval requests?
in O365, you can use the Assign a Task action to assign a task to the user. This action will by default create the task in the Workflow Tasks lists.
Now you need to go to the Workflow Tasks list, click on Nintex Forms and customize the form there.
On the form design, you'll be able to add a rule that makes it a requirement that the comments field be filled in if the user selected Rejected.
If I understand it well, you want to make sure the Comment field is validated if the manager Rejects.
thanks for the reply. If you can give me a bit more info about how i can do this that will be great. I am very new to this Nintex workflows.

I have tried it out and the trick is to find the real value for the 'Rejected' value.
At the end of the post, Andrew Glasser found the following that helps me to make it working.
To verify, he did View Source to the page and search for 'data-nfChoiceValue' for the selections for the Task Outcome column. This will give you the value you need to test on.

there is no request approval action in 0365 Nintex 2013.
I am confused. I tought you were using the Nintex Workflow action 'Assign a Task' and you wanted to customize the Task Form with Approval and Reject Task Outcome.
If not, can you explain your sentence from your original post 'when they reject an approval requests'
yes that's right...i am using "Assign a Task" action and i am confused with the task form. How can i customize it?
In the List Ribbon, you must see the button Nintex Forms. If this is not the case make sure to Install the App if O365 or Activate the feature if on-Premise.
After clicking the button, you will see the UI to customize the form.
the article saying drag a request approval action from user interaction but i can't find such an action in my nintex workflow.
If you cannot see this action, it means that your are using O365.
In that case, use Assign a Task.
yes i am using O365. I used assign a task action and when we assign a task to manager is there any way to enter comments by the manager when she rejects the approval.
As explained above, the validation must happened in the Task Form.
In the Nintex Task form, create a Comment field and follow the instructions
that really saves a lot of time. Thanks for your help. I have managed to get the user to enter the comments when they reject an approval(and task outcome value is not 1 or 2 its "Approve" & "Rejected") but the problem now is when user approve or reject any requests they are not getting any notifications. In my workflow I have created a variable called approved and set the value as 1. on condition check I put workflow variable approved=1 for approved and send email to the use. But the user is not getting any notifications after I customize the task form.
Any help with this plss...

yes I used conditional branching
it was working before and after I customized the task form it stops working..
place some Log in the History List actions in your workflow and then publish and run the workflow.
See where the workflow is going. It may not be going down the branch that you expect that sends the email.
The values may have changed for the Approve Reject options in the HTML. Can you view source of the form and print here the html for the radio selections?
I have no luck working with this. I am not sure about the working of the form coz i am very new to this nintex. can you help me? I have a change request form that should go to the manager and approve it. But the requirement is that she wants to enter comments when she rejects the form. So i create a change request list and Workflow diagram is as follows
it is working fine before i customized the task form. But when i customized form i am not able to receive any approval email. I am lost should i create any workflow in the workflow task item.?. this is the view source for the task form and no workflow in the workflow task items
Help clarify something for me, it sounds like you have two issues.

I have already tried this to enter comments on rejection and it works well but after I did that its not entering the approval or reject loop and sending emails...ITs not only about this , I just tried a simple one by customizing the task form. After customizing the task form its not sending any approval or rejected email notifications. I am not sure how to debug this?
Vadim's suggestion of adding some 'Log to History' actions will help you trouble shoot any issues and also give you a path that the worklof followed. If you example, I would add 3 actions based on the image below with the following messages:1. Task Approved
2. Task Rejected
3. Task and Email Notification Sent.
When the workflow runs, you'll see the path it took, or didn't take, based on what shows up in your history log. If you find out that the workflow is moving through a branch and still isn't sending an email. I would start to use workflow variables. Before the first email, set the variables equal to the field the email was pulling the user name from. You can then include the variable in the 'Log to History' to see what the value that is.
The 'Log to History' is an action that should become apart of all workflows as it moves you towards a best practice.
I am very new to this nintex workflow and not familiar with the history list actions. I tried to enter the history list message for approved as approved and rejected as rejected. But i dont know how to trigger the values where do i look the values after running the workflow.
