Document Generator with Excel output

I created a Document Generator action in a workflow that is supposed to populate columns in an Excel doc from a custom list using a Nintex Form. I have been able to set the names on the Excel spreadsheet so that they are accurate but the only column being populated is from a standard SharePoint column Created By and the other columns attached to controls on the form-most of which are Single Line Text boxes-are not populating. I think I am missing something possibly in the workflow.
