Need Help on look up columns save

HI, I am developing a Nintex form in office 365, in that I have two list lookup columns, 1. State and 2. Office address When a user select state from the droop down which is connected in a list the second office address will get populated based on the selected state its working good, but when I save the office address will not get saved and when I edit the saved form it is displaying the office address since it is loading based on the state selected, but in View form it is not showing anything in the office address. Can someone help me to solve this problem. Thanks, Rajan.


also i try to load the Input below the selection using javascript, but that is also not working.
