I use the 'request approval' action often just as a task not as a review. Is it possible to delete the sentence with the link 'Use this page to review submissions. Learn about requiring approval.' at the top?
I think that, if you cannot use any tool to customize the task form (Nintex Forms or Infopath), then I think that the only solution to do what you want is to create a different page that needs to be used when editing the task.
By default, in Central Administration - Nintex Workflow - Global Settings there is a property called "Default task edit page" that refers to _layouts/15/NintexWorkflow/ApproveReject.aspx . I think that you need to create a page similar to that, unfortunately, because it's in _layouts/15/NintexWorkflow it isn't accessible from UI or from SharePoint Designer, and honestly I don't know if there are any issues if you create a copy of the file from the server and do your changes on the copy..
