How to build a form to allow for unknown number of expense claims

Hello, are there any examples around ( I have searched ) that show how to build an expense claims form that will allow someone to just keep adding items to it?

Say they need to claim for 6 different items, is there a way we can just keep adding a new row of data fields for the item and cost?

The form can default to 1 item, but what if they need to add more items?
