List that exists is not found when using a Lookup

Issue

An existing list is not found when using a lookup on from another Site Collection.

 

Resolution

Ensure that the root site exists in the Web Application.

Navigate to Central Administration > Application Management > View All Site Collections > Your Web App
 
Note:
The root site will look similar to:
http://my-server.domain.com/
Notice that on the root site, nothing follows /.
