SharePoint/Nintex on prem 2013

A workflow associated with a workflow task list has been working perfectly fine until recently. It was noticed to show 3 duplicate times when you click on the ellepsis beside the item to start the workflow manually.

when I choose 2 of those options, I get the following error. Checking the logs with the corrlation ID I don't see anything useful... how can I remove those duplicate options? and why is it showing that error? The workflow is only created once; i.e. there is no multiple workflows with the same name associated with this list - you know, SharePoint won't even allow for same name workflows. any ideas?
