I use Nintex Workflows for SharePoint 2016 on-prem. Since past couple of weeks I have been getting "Workflow_Name failed to run" error for a few workflows with "Assign a Flexi Task" action. I get this error when the concerned users don't respond to the Flex-task action for more than 3 days and the workflow outcome changes to "Canceled". If they approve / reject the request within 3 days, the workflow runs smooth without any errors.

The 3 day time frame sounds pretty strange in this one, but looking at the intervals on the errors (every 5 minutes), it looks like an issue with the workflow processing via the Workflow Timer job on Application Servers without having the proper config setup. I would try starting with the troubleshooting recommendations in the articles below, and verify if you still see these issues.
