Get last choice responses auto populated while filling the form

Hi,

I got lot of help from this community so far. Thank you everyone :)

I have unique requirement to implement in one of the forms. Below is the scenario:

Review List: List where all responses are saved for each Opportunity ID every month. User fill the form based on this list every month. so, every month a new entry gets generated for a specific and unique Opportunity ID.

Master List: This list is unique. Each Opportunity ID will have only one entry. I have created a workflow to update the master list with latest entries from Review list responses.

Requirement: I need a way, where when the users filling the form (based out of Review list) should see the choice fields auto populated with their last responses. So that it will be easier for them to fill the questionnaire.

 

Please help.

Please understand that the field I want auto-populated is a choice field.

Still waiting for a solution on this.

Any javascript or rules that I can use to achieve this is really helpful.
