Clicking on instance dirrects you to the wrong instance

We have a K2 2003 server that has been running fine for about a year. Recently we are seeing some users experience an error. When the click on an item in a report, they are taken to the wrong item. For example lets say I have a report on an open task with the Folio of XYZ and you click on the task, it actually opens up the task for ABC .

Everything else seems to be running fine, but the worklist misdirection is starting to cripple our ability to manage K2.
