In Nintex Workflow on premises, you could add outcomes to your task and set these with descriptions and whether they were required.
Is there any actions to still do this online? Or do we need to create a Nintex form for the task and add custom validation?

In o365 tasks, you can define custom outcomes as well as edit the task form, then using rules to require comments.
