I truncated the data in my SmartObject through SQL Server Mgt Studio while there is still an open request in my process. I can still see it in my worklist and tried to approve and reject it but nothing's happened. And its status is still Active. Is there a way to permanently delete that request?

I believe your request is nothing but WORKLIST.
Worklist can be deleted permanently by stoping the instance in K2 Workspace. Follow the below link to navigate to the Process Instance in K2 Workspace.
