Symptoms
The smartform is designed as such - form A contains form B. When executing the rule "get confirmation from user", a message box will appear which includes a ok and cancel button.
The issue is that the cancel button was not in a proper position of the box.
Diagnoses
Occurs only when using the lithium theme, other themes have no issues.
Resolution
This is a known issue on smartforms 4.6.9 and has been logged as a bug to be fixed in a future release.
As a workaround for now, please use a different browser such as Chrome and Firefox to resolve the issue.
