How do I confirm that 'lazy approval' emails are operating as expected?

They are getting hung up in the SMTP server folders and SharePoint is reporting an 'unrecognized' alias. This was all working fine until recently.

 

Exchange team reports no modifications to the AD/Exchange object.

 

Large environment with many 'lazy approval' users.

 

Do not want to have to reset the settings.

Outgoing lazy approval emails or the response emails?

Outgoing and response emails must have [#DDDLASDASDD#] or something like that in the subject when replying

Inbound. It is getting caught up in the processing by the SMTP server in SharePoint
