I think you changes permissions on library item not on document itself.

That describes the behavior I'm seeing so I think you are right ...so how do I get the item permissions to translate to the item's document?
you have to choose your docu library and define condition for the document(s) you want to change permissions for
something like

[Solved] Top Level Site Admins see all things at all times, no matter how permissions are set.
