Sending notification to initiator when approved/rejected

you can achieve this by picking the initiator from the workflow context. Following screenshot shows the example in SharePoint on premises. Should you work with Office 365 the UI is a bit different but the approach is the same:

I assume you have something like this:

When double clicking the notification action you can configure the to field:

Head over to the lookup section and pick initiator by double click

Once you close the dialog you should see the initiator placeholder in the to-line, which will be enough to send a message to the person that kicked off the workflow
