Nintex Forms for Office 365 Dialog Issues

We have a Nintex form on a list that opens in a dialog, and for some users, it won't load properly:

This is happening in both Chrome and IE11, but not consistently for all users.

The forms will open normally without any issues when opened in a new tab, and only seems to be an issue when opening in a dialog.

Has anyone come across anything similar?

Have you any script error on the IE status bar?
