How to set a workflow for document library?

Hello,

I created a document library in sharepoint site, and now I want to set a workflow for approve, the question is:

1. I want to display the workflow start form when user stop edit a document, then user will input something to start the workflow

2. In the workflow start form (or approve form), I want to display the document with hyperlink and (...), like the following picture

What should I do for this?

I think you cannot add the 3 points menu there.
