What your screenshot is showing as "Assign to Facilitator for Approval"  is actually a "Assign a Flexi Task" action. It seems like somebody changed the description.

1) Request Approval action only allows two options--> Approve or Reject. Assign Flexi Task allows you to specificy your own outcomes and take necessary action based on the outcome.

2) Assign Flexi Task also allows you to add reminders and escalation.

Do you know how to make the workflow outcomes post to my data collection list?

For example any approver comment the approver makes when responding to an email or whether the task was approved or rejected?

The easiest way to do this is simply create a column called "Approval Status" (or whatever you choose) and in each of the branches Update the Item with a string saying "Approved", "Rejected" or "Review" (again, whatever you wish).

You could also use the "Set Approval Status" which sets the built in SharePoint Approval column status, but that can only be set to certain options like, pending, approved or rejected.
