Error while choosing Docusign as Delivery Option

I am trying to add the DocuSign as the Delivery option and trying to send the Document for ESigning. But I am getting the below error

The username or password for the requested third party service (Salesforce) could not be found. Please have your administrator contact support. (ExternalAuthMissing)

I added the Docusign service as th Third Party Integration like below (not sure if the Bae URL is right)

And in the Document Package I gave the Username and Password which I am using using to log in to the https://appdemo.docusign.com/ URL 

But when the DDP runs I am getting the error as shown above. Am I missing something here?
