I just want to make sure I am doing this right. I have a library with an approval workflow and all 3 approvers are expected to approve a document or otherwise it is rejected. Shoud I use the following settings:
Completion criteria: Wait for percentage of a resposne
Required percetage: 100%
Required Outcome: Approved
Default Outcome: Rejected

That should work, and would be the best route to complete it.
Another option you could use would be setting the completion criteria to 'Wait for Specific Response' with the required outcome of 'Rejected' and a default outcome of 'Approved.' This would wait until someone rejects the task, otherwise it will go down the approved branch. Your setup would work better though if you are going to have additional branches within the task (more info, other etc.). This configuration would look like the setup below:
