Symptoms
After Upgrade environment from 4.6.6 to 4.6.11, and ran the DB consolidation tool, environment seems to be functional but the process instance reports are broken. When testing the process instance smartobject it fails with,

Object reference not set to an instance of an object.

Diagnoses
The issue was caused by missing values for smartforms environment fields.
Resolution
Updating smartforms environment fields for this environment with the correct value, resolved the issue.
