This Office 365 form is the starting point for creating a new vendor assessment.  The user can choose to enter a new vendor or select an existing one.  If an existing vendor is chosen, the user can select an Assessment Type and the "Find a Vendor" drop down filters with only vendors of that type to choose from.  Selecting an existing vendor auto populates the vendor's contact information in all fields in the "Please provide the Vendor's contact information".
